For each statement, choose the response that best describes how often it applied to you during the past two weeks.

1 I found it difficult to stop worrying once worried thoughts began.
2 My body felt tense, restless, or unable to fully relax.
3 Anxiety made it harder for me to concentrate on work, study, conversations, or other tasks.
4 I felt unusually alert for signs that something might go wrong.
5 I avoided a situation, activity, person, or place because I expected it would make me anxious.
6 My mind repeatedly moved toward possible problems or negative outcomes.
7 I noticed uncomfortable physical activation when anxious, such as a pounding heart, shakiness, sweating, or rapid breathing.
8 Anxiety interfered with my ability to rest or sleep well.
9 I experienced moments when fear or anxiety rose suddenly and felt difficult to control.
10 I relied on reassurance, checking, preparation, or other 'safety' behaviours to feel less anxious.
11 Even when I wanted to switch off mentally, anxious thoughts kept returning.
12 I felt physically keyed up or on edge without being able to settle easily.
13 Anxiety negatively affected my work, studies, relationships, social life, or everyday responsibilities.
14 Situations that were not clearly dangerous still felt threatening or unsafe to me.
15 Anxiety influenced decisions I made or stopped me from doing things that mattered to me.

Your total score, its descriptive range and your five-area profile are shown once every statement has been answered. Skipped statements would make the total meaningless, so the questionnaire has to be completed in full.

Where should we send your result

Enter your name and email to see your result. Your responses are health-related information: they are stored only by this practice and a copy goes to your therapist so it can be read before a first session. Nothing is shared with anyone else.

Understanding your result

Your score is intended to help you reflect on what you are experiencing. It does not provide a diagnosis. A higher or lower score cannot by itself confirm or rule out an anxiety disorder.

If anxiety is causing distress, affecting your daily life or relationships, or preventing you from doing things that matter to you, a professional conversation may help you understand what is happening and what could help.

You are welcome to discuss your results with me in a confidential initial consultation.
